Is tsubasa chronicles really the continuation of card captor sakura?

Cause many people told me that there was a time in the manga that the real sakura and syaoran was actually mentioned

But yes, Tsubasa and CCS' stories are somehow linked.
The Sakura Kinomoto (Cardcaptor) meets with older Sakura (Tsubasa) in her dreams. Sakura Kinomoto gives her staff (The ngôi sao wand) to Sakura, saying that 'You know, everything will be all right'.
The staff ended up being a payment for Yuko later. But that's a whole different story.

As for the Syaoran in CCS, I don't think he made an appearance, though.
