I'm pretty sure she would be his medical proxy, yeah. She's usually either the proxy, or his attending. Either way she's got some form of control =D

When he was on the stretcher, he told Cameron to tell Cuddy that he wanted Ketamine, so House did specifically request Cuddy, so that pretty much confirms that either they had agreed some time in the past that Cuddy would the proxy, OR that because House requested Cuddy at the time of his shooting, it was decided that she would be in charge. I think the latter is more likely.

He hallucinated or dreamed the whole ketamine thing - almost the whole ep was unreal. She didn't do anything out of her own will. It wasn't real. The end of the episode shows him waking up, he had just been shot, they are rushing him to the OR, and he says: "tell Cuddy to give me ketamine". He asked for it. She didn't make any decisions in his name.

But perhaps in his subconscious, she was his health proxy... After all, in his hallucination, she gives him the ketamine without his consent, which must mean she had authority over the medical decisions that concern him.

However, i think that this hallucination was House's way to make some peace with the past, with Stacy deciding to operate on his leg and Cuddy playing along. In the hallucination, he explores the same type of anger he felt back then, but when he wakes up, he decides to take the chance and get the ketamine.

DB good point on the whole thing but he specifically said Cuddy not Wilson or even Cameron to tell the surgeon ( when he was rushed in for real)

Well, yes, but that makes sense. First of all, because the leg issue is first and foremost between him and Cuddy (in the absence of Stacy, that is). She was his attending when he got crippled, he has always blamed her for it on some level and she has always felt responsible for his leg.

Second - the way his subconscious made the connections perhaps suggests that the ketamine thing was previously discussed by House and Cuddy. It's not impossible that her guilt led her to search for solutions to improve his condition, and she has mentioned them to House through the years - and she got blown off, of course. If she had suggested ketamine to him before, then it's crystal clear why he hallucinated that particular scenario, and why he asked for Cuddy to give him the ketamine, and why he assumed she would know exactly what he was talking about.

Third - she's the boss - so anything she tells the surgeon is gonna be less of a subject for discussion - perhaps Wilson or Cameron would be more likely to be asked for consent and health proxy authorization, but Cuddy - less so.

I always thought it was interesting that on any other show you would have the shot of the gurney going down the hall and the character would say "tell _____ (female romantic interest) I love her. Here we have House saying "Tell Cuddy-----------I want ketamine." Then season 3 we have Cuddy trusting him (first episode with the paralyzed patient) lying for him, and the characters getting closer, House getting jealous. etc.

Well Cuddy is his medical proxy, so it makes sense that he asked her for the ketamine. She was also his attending in season 4 when he went into the coma and he went to her for help in his hallucination, so I think its safe to assume that he trusts her with his life :)
