In fact it's quite tricky, because the question is: Are you the only who has a crush on Clegane? And this True sort of means Yes, you are the only one, while Not really means that people share your feelings.
But somehow it appears that True! is the one who has a crush on Clegane, like you and Not really is the one for those who do not really share your feelings.
So I'm not sure I picked the correct option, but my answer is no, I don't have a crush on Sandor Clegane at all.
